Appointment Cancellation Policy

Holy Family Mecial Associates, LLC, will be available by phone from 8:30 a.m. to 4:45 p.m. Monday through Friday. If you are unable to make a scheduled appointment, please call your office as soon as possible. A 24-hour notice for cancellations is preferred; this allows us to satisfy the requests of other patients who are seeking timely medical care. If you are late or don’t have the appropriate information for your visit, it may be necessary to reschedule your appointment.

Extended Care Clinic

Holy Family also offers Extended Care hours, early morning urgent care for established patients. Extended Care hours are 7 to 8:15 a.m., Monday through Friday and 9 to 10:30 a.m. on Saturday.

No appointments are necessary to take advantage of the Extended Care hours. Please note that if you arrive for our Extended Care program nearing the end of our set hours, we may need to schedule an appointment for you instead.

Complete Physical / Preventative Visit Appointments

If you are scheduling for a complete physical/preventative exam with one of our physicians, please note that this visit must be billed to your insurance company as a physical/preventative measures that you and your provider feel are appropriate for your age and medical condition.

If you want o talk about additional problems or concerns today, your physical/preventative visit may need to be rescheduled to address those medical concerns separately. It is important to note that insurance will not allow physicals/preventative exams to be billed with any additional problems.

When documenting the detail of your visit, providers assign specific codes according to the services he/she provides. It is the patient’s responsibility to check with his/her insurance carrier to confirm that physical/preventative services are covered.
